• 5. Do I engage in my businesses considering whether my acts are ethical?
• 6. Do I engage in my businesses following the procedures and methods prescribed by laws and regulations
• 7. Am I aware of the company’s principles and guidelines related to the ethical business management?
• 8. Have I been unfairly treated, because I refused any unfair order of my superior for the last one year?
• 9. Has any officer or employee of my company made others to serve drinks or dance at company gatherings, unpleasant physical contact or obscene comments?
• 10. Have I received any valuables or been entertained by any stakeholders from outside the company during last year?
• 11. Have I used the company credit card for personal use during last year?
• 12. Have I seen any officer of employee of company providing or receiving valuables or entertainment (which cannot be deemed as an ordinary one) to or from any stakeholder, or received or provided such valuables or entertainment myself?
• 13. Have I illegally or improperly spent company’s funds?
